Four patterns carry nearly all of breathwork's controlled evidence — one for the spike, one for the night, one for steady attention, one for training. This page opens the toolbox: the counts, the evidence behind each, and the gaps where popularity runs ahead of proof.
What the evidence supports
- All four patterns share one active ingredient: an exhale at least as long as the inhale — the lever the physiology page dissects.
- The physiological sigh has direct randomized-trial support for mood and physiological arousal — the strongest single-technique evidence in the family (Balban et al., Cell Reports Medicine, 2023).
- Slow breathing near 6 breaths per minute reliably increases heart-rate variability across dozens of controlled studies.
What remains uncertain
- 4-7-8 and box breathing each have only a handful of controlled trials; much of their reputation runs ahead of the evidence.
- Whether any one pattern beats the others over months is largely untested — direct comparisons are few and short.
- How much of each technique's benefit comes from the pattern itself versus the counting, attention, and quiet that surround it.

Four Moves, One Lever
The Breathwork parent topic introduces the family and the honest boundaries around it; this page goes pattern by pattern. The four techniques differ in counts, timing, and intent — but they all pull the same lever, an exhale at least as long as the inhale. Learn one and you have a working brake. Learn the differences and you can aim the brake at a specific moment, which is exactly what When to Use Which walks through.
- 😮💨 The physiological sigh — two short inhales through the nose, one long exhale through the mouth. Built for speed: one to three sighs, seconds to deploy.
- 🌊 4-7-8 breathing — inhale 4, hold 7, exhale 8. A wind-down pattern designed to be practiced lying in bed.
- 📦 Box breathing — 4 in, 4 hold, 4 out, 4 hold, like tracing the sides of a square. An even, focus-first rhythm.
- 🎚️ Resonance breathing — about 6 breaths per minute, roughly 5 in and 5 out, no holds. The training pattern, not the rescue pattern.
4-7-8 Breathing: The Sleep-Onset Classic
The pattern is attributed to Dr. Andrew Weil, who adapted it from pranayama: inhale through the nose for a count of 4, hold for 7, exhale through the mouth for 8. The arithmetic is the design. The hold builds a mild, comfortable air hunger, and the 8-count exhale runs twice as long as the inhale — the long-exhale asymmetry in its purest form. A full cycle takes 19 seconds; a handful of rounds spans a couple of minutes.
The direct trial evidence is modest. In one controlled study, healthy young adults did 4-7-8 practice after a night of partial sleep deprivation; the technique improved heart-rate variability and blood pressure relative to controls (Vierra et al., Physiological Reports, 2022). Beyond that, support is indirect: 4-7-8 is a slow-breathing pattern, and slow breathing's general effects are well documented in systematic reviews (Zaccaro et al., Frontiers in Human Neuroscience, 2018). Its popularity for sleep onset is real; its specific trial base is thin. Two honest caveats follow.
- 😤 The hold is optional. If holding for 7 creates tension or air hunger that feels like effort, drop it and breathe 4 in, 8 out. You keep the lever.

Box Breathing: The Even Rhythm
Four equal sides: inhale 4, hold 4, exhale 4, hold 4 — a 16-second cycle, symmetrical where the others are lopsided. Box breathing entered popular use through military and first-responder training, where it is taught as a composure drill before high-stakes tasks. The proposed mechanism is attention rather than gas exchange: counting four equal phases occupies the mental bandwidth a spiraling thought would use.
The evidence is thinner than the fame. In the head-to-head trial, box breathing did improve mood — but less than the physiological sigh did (Balban et al., Cell Reports Medicine, 2023). No large trial has tested box breathing for performance or focus specifically. That does not make it useless; an even, counted rhythm is a reasonable composure tool. But the honest label is "plausible, largely untested," not "validated." If the goal is calm, the long-exhale patterns carry the better evidence. The Physiological Sigh: Fastest to Deploy
Two inhales stacked with no exhale between — a normal breath, then a top-up sip of air — followed by one long, unhurried exhale. The pattern is not a wellness invention; it is a reflex that runs automatically roughly every five minutes in animals and people, and the brainstem circuit that drives it was mapped directly in mice (Li et al., Nature, 2016). The double inhale reopens air sacs that shallow breathing lets collapse, and the long exhale applies the brake the physiology page describes.
The trial evidence is the strongest in the family. In the 2023 head-to-head, over one hundred healthy adults practiced one assigned pattern for five minutes daily across a month; the sigh group showed the largest gains in positive mood and the largest drop in respiratory rate of any group — including the mindfulness-meditation comparison group (Balban et al., Cell Reports Medicine, 2023). The protocol is short enough to remember: inhale through the nose, take a second short sip of air, then exhale slowly through the mouth until the lungs feel empty. One to three sighs in an acute moment; five minutes daily as practice.
Resonance Breathing: The Training Pattern
Six breaths per minute, roughly 5 seconds in and 5 seconds out, no holds. Where the sigh is a rescue and 4-7-8 a wind-down, resonance breathing is training: it flexes the baroreflex loop at the frequency where heart rate, blood pressure, and breathing lock into a single amplified oscillation (Vaschillo, Vaschillo & Lehrer, Applied Psychophysiology and Biofeedback, 2006; Lehrer et al., Applied Psychophysiology and Biofeedback, 2000). Biofeedback protocols use sessions of 10 to 20 minutes, most days, often with a pacer. A systematic review of slow-breathing research concluded the practice consistently increases heart-rate variability and markers of vagal activity (Zaccaro et al., Frontiers in Human Neuroscience, 2018). The resting-state changes — the part that shows up on a wearable as a higher morning HRV — take weeks of regular practice. Treat it like any training stimulus: consistency first, then weeks, then measurement.
🧰 A technique is not a treatment
These patterns manage autonomic state; they do not treat disease. If 4-7-8 has become a nightly requirement, the pattern is managing a symptom — undiagnosed sleep apnea or chronic insomnia deserve an evaluation, not another round.

Choosing Without Overthinking
- 🛟 For the spike, the sigh. One to three physiological sighs settle an acute moment in seconds — the strongest trial backing for a single move.
- 🌙 For the night, 4-7-8. In bed, lights low, four to eight cycles — or the plain 4-in/8-out version if the hold feels like work.
- 🎯 For composure, box. Before a talk, a difficult conversation, a test: a minute of even counting, and the label stays honest about its evidence.
- 🏋️ For the long game, resonance. Ten to twenty minutes most days; expect the resting-HRV payoff over weeks, not days.
- 🧭 None of these should feel like straining. Lightheadedness or tingling means you are over-breathing — stop, breathe normally, and read the hyperventilation caution before continuing.

Questions, Answered Briefly
- 🧘 Which one should I learn first? The sigh — it needs no counting and has the best evidence. Add resonance breathing as your training pattern once the sigh feels automatic.
- ⏱️ Do I have to hold for exactly 7? No. The counts are scaffolding for the ratio, not a performance standard. A 4-in/8-out breath keeps the lever without the hold.
- 📅 How often? Five minutes daily was the tested dose for the sigh. Resonance training fits 10 to 20 minutes most days; rescues happen as needed.
- 💪 Do longer holds mean faster results? There is no evidence for that, and forcing holds is how people drift toward hyperventilation symptoms. Longer is not the goal — the ratio is.
