Estrogen is not just a reproductive hormone; it is a structural one. It slows bone resorption, and it holds the lipid profile in a favorable shape — which is why premenopausal women tend to carry lower LDL than men and why cardiovascular events in women lag roughly a decade behind men's. When estrogen falls through the transition, both protective effects unwind together, and two risk curves bend upward in the same decade. The Heart Side: What the Transition Does to the Numbers
The Study of Women's Health Across the Nation — the large cohort that followed women through midlife — found total and LDL cholesterol climbed through the transition even after age was accounted for, meaning the change is not simply "getting older" (Matthews et al., JACC, 2009). HDL drifted downward; modest per year, the shifts compounded across the transition's five to ten years. The screening consequence: a mid-40s lipid panel is not a permanent certificate. The Bone Side: The Silent Erosion
Bone is constantly remodeled — resorbed and rebuilt — and estrogen is the brake on resorption. When the brake releases, the fastest loss happens early: roughly 2–3% per year in the first postmenopausal years, tapering toward 1% and below as a lower equilibrium settles in. Over the first five to seven years, the spine and hip can give up a meaningful fraction of their peak mass, which is why a fracture that would have been a bruise at 45 can be a hip fracture at 75. The screen for all of this is the DEXA scan, timed by the incidence curve: fracture risk climbs steeply only after the mid-60s for average-risk women, so the USPSTF's 2025 statement puts the first scan at 65 — earlier for women with early menopause, low body weight, smoking, heavy alcohol use, long-term steroid use, or a prior fracture. One caution from the trials: bone loss is not fixed by a supplement alone. In the VITAL trial, vitamin D3 at 2,000 IU daily did not reduce fractures in healthy midlife and older adults (LeBoff et al., NEJM, 2022) — the levers that move bone are load-bearing exercise, protein, and clinician-prescribed treatment where indicated.
Reading the DEXA: T-Scores and FRAX
A DEXA report has two layers. The first is the T-score, your bone density against a young-adult reference: a T-score of −1.0 or better is normal, between −1.0 and −2.5 is osteopenia, and −2.5 or below is osteoporosis. The second layer matters more for decisions: the FRAX tool folds your T-score together with age, weight, fracture history, family history, smoking, and other inputs into a ten-year fracture probability — the odds of a hip fracture and of any major osteoporotic fracture over the next decade. Treatment conversations typically open when ten-year hip-fracture probability reaches about 3%, or major-fracture probability about 20% — where treatment's fracture-prevention benefit outweighs its risks for most women. Two women with the same T-score can land on opposite sides of those thresholds, which is the point: the score locates you, and the probability decides.
The Lipid Panel and the Risk Calculator
The heart side has the same two layers. The lipid panel gives the raw numbers; the Pooled Cohort Equations — the ACC/AHA calculator — convert them, plus age, blood pressure, diabetes, and smoking, into ten-year cardiovascular risk. The guideline conversation threshold sits around 7.5%: above it, a statin discussion is recommended; between 5% and 7.5%, additional factors tip the decision. For women, one of those factors deserves special mention: the coronary artery calcium score — a low-dose CT that directly images the plaque the calculators only estimate. A score of zero in a midlife woman reclassifies a surprising number of "intermediate-risk" patients downward — sparing a decade of medication — while a high score does the reverse. It is the imaging tiebreaker the heart side has and the bone side lacks.

Why Screen Them Together
The pairing is not just biological; it is practical. First, the timing: the transition reopens both folders in the same decade, so one visit rechecks the lipids and starts the bone conversation. Second, the interventions overlap — the strongest evidence-backed move for both is resistance training, which loads bone at the hip and spine while improving the metabolic and cardiovascular profile that the lipid panel measures; the strength topic owns the dose. Third, and least comfortably: both are silent until the event, which is where most women first learn of either. A fracture at 70 and a first cardiac event at 65 both carry the same message in retrospect — the numbers were there to be found. Screening both, on schedule, in the same visit, is the cheapest insurance against that retrospect.
