Cognition: A Real Signal on Young Evidence

The brain uses creatine the same way muscle does, so the cognitive trials were a natural next step — and they found something, with caveats attached. A systematic review of randomized trials in healthy adults (Experimental Gerontology, 2018) found creatine improved performance on memory tasks, with results in other domains more mixed; the clearest signals appeared under conditions of stress — sleep deprivation, demanding workloads — and in people with lower baseline stores, a finding the brain health review by Forbes and colleagues (Nutrients, 2022) frames through the energy-demand mechanism. Bone: Early but Interesting

Bone cells run energy-demanding remodeling processes, which makes creatine a plausible — but largely untested — lever. ⚠️ The one population that checks in first

For healthy adults, creatine's safety record is the most thoroughly documented of any supplement discussed on this site. The exception is kidney disease: because creatine raises creatinine — its normal breakdown product — anyone with impaired kidney function should only use it under clinician guidance, both for safety and so that routine lab values stay interpretable.

Mood and the Frontier

The newest work asks whether brain energy buffering helps mood disorders. Small trials have tested creatine as an add-on to antidepressant treatment, with a few positive signals in women and mixed results elsewhere, and the Forbes review (2022) lists depression, traumatic brain injury, and neurodegenerative conditions as areas where the mechanism is plausible but the trials are preliminary. This is the hypothesis stage of the literature: worth watching, not worth acting on. The same honesty applies to the popular hair-loss worry, which traces to a single small study that measured a hormone ratio in rugby players during a loading phase — it never measured hair, and no trial has linked creatine to balding.
